Healthy Weight Nursing Team
BarnetProvided By
Weight management support for children aged 4-12 who are above the 99.5th Centile.
The service offers:
- Face to face and telephone support.
- In-depth nutritional planning.
- Guidance around diet and exercise.
- Support to improve healthy lifestyle.
Referrals are accepted from:
- Self (Family member/child/carer).
- Other professionals such as school nurse, GP, physiotherapist, occupational therapist, SENCO, schools.
- Other agencies such as social services, youth workers and voluntary organisations.
A referral form must be completed after consent from parents/carers.
Eligibility Criteria
Inclusions
- children who attenda mainstream school or have a GP in Barnet
- haveno complex needs or other underlying health conditions
Exclusions
- children who reside and have a GP outside of Barnet
