The aims of the Whittington Health Enhanced Health in Care Home Team (WH EHCHT) are to:
- support primary care to fulfil the community healthcare requirements outlined in the national WH EHCHT framework in older people’s care homes in Haringey
- provide in-reach multi-disciplinary support to the identified homes.
The WH EHCHT model:
- delivers high-quality personalised care within the homes including transition to/back to home from hospital, comprehensive assessment, supporting development of and implementation of proactive care plans for residents
- provides the right care and the right health services for the individual where possible
- reduces crises and resulting unnecessary conveyances to hospitals, hospital admissions and bed days whilst ensuring the best care for people living in care homes.
The WH EHCHT model supports Haringey learning disabilities partnerships and BEH MHT in their role as leads for managing their individual cases, as well as in-reach into mental health and learning disabilities care homes if the identified needs are associated with frailty.
Eligibility Criteria
Inclusions
- open to West Haringey care home residents (Springlane, The Meadow, Brownlow, Alexandra Park and Ferview (South and West homes))
- the service also supports Haringey learning disabilities and BEH MHT mental health partnerships in the mental health and learning disabilities homes
