The Enfield Community Matron and Anticipatory Care Planning services run alongside each other.

Community matrons are senior, experienced nurses and autonomous practitioners with enhanced skills, including:

  • independent prescribing
  • physical assessment
  • advanced care planning.

Community matrons facilitate and promote independence in the community by proactive case management, minimising inappropriate and unplanned hospital admission/attendance.

The Anticipatory Care Planning service provides health and wellbeing support for people earlier in their healthcare journey/diagnosis, enabling people to better self-manage their health and improve wellbeing.

Anticipatory Care Planning team members include a mental health nurse, case finders, care navigators and peer workers who have lived experience.

The Community Matron and Anticipatory Care services hold regular multidisciplinary meetings and work closely with other healthcare professionals, GPs, social services and voluntary sectors.


Eligibility Criteria

Inclusions

  • individuals with long-term conditions who have recently needed emergency hospital treatment
  • individuals who frequently consult their GP
  • individuals who have health and support needs.

Complete the Adult Services Single Point of Access Referral Form - Enfield Community Services and send to the centralised Enfield Community Services address e: beh-tr.ecsadultsinglepointofaccess@nhs.net

Once the referral has been received, it will be screened by non-clinical administrators with no changes to clinical response times.
