Child Weight Management Service
HaringeyProvided By
The Child Weight Management service for patients who have been identified as overweight or very overweight. This service offers patients and their families support in the form of interventions which are provided by a variety of voluntary care sector organisations.
Our Goals:
- To address prevalence of obesity in children in the east of Haringey through education about healthy lifestyles for both children and families.
- To offer support over a six-month period for any child and family that engages with the service.
- To build relationships with local community providers and organisations.
Eligibility Criteria
Inclusions
To meet the eligibility criteria, patients must be:
• Aged 5-17.5yrs
• Registered at a practice from one of the four PCNs: N15, Welbourne, North East or East Central
• 91st BMI centile or higher (overweight or very overweight BMI)
Exclusions
Anyone who doesn't meet the above eligibility criteria
How to Refer
The service is open to referrals from primary care, secondary care, schools, school nurses and self-referral

Complete the Child Weight Management Service Referral - Haringey GP Federation and send to nclicb.childweightmanagement@nhs.net
Where to find the form
- Haringey: HAR Global Documents > Paediatric
For proactive referrals: A search has been created on EMIS that will pull a list of patients for the Care Coordinators to work on, this list will be pulled on a quarterly basis to ensure any newly coded patients be picked up as well.
