Camden Community Mental Health Services: NLFT

As part of North London Foundation Trust's (NLFT) community transformation programme to improve the coordination and continuity of care, the Rehabilitation & Recovery (R&R) teams have now been integrated into three locality-based Core Teams. These changes have already gone live, and service users previously under the R&R teams are now being supported by their aligned Core Team.
There are three Core Teams across the borough of Camden:
- Camden North West Core Team
- Camden Kentish Town Core Team
- Camden South Core Team.
The core teams work closely with GPs, specialist services (including secondary care mental health services), housing and social services.
The focus is on relationship-building, shared responsibility and collaborative working with individuals, their networks and the systems around them, supporting people to find purpose in their lives, expand structured meaningful activities and engage with community-based groups and offers.
Core teams work may include:
- mental health assessment and advice
- pharmacological treatment
- brief psychological therapies
- social care and inclusion
- peer coaching
- psycho-social support including from Voluntary Sector providers
- holistic health and wellbeing support
Using a trauma-informed approach, a holistic personal care and support plan is co-produced with individuals. A named staff member is a point of contact. This is a short-term service; should long-term support be required then the team can refer or suggest alternative services.
Based on the patient's GP practice, the care will now be delivered from the following locations:

Self-referral
The crisis line is open 24 hours a day, seven days a week. If in crisis, patients should ring the number above or contact the usual mental health team who can advise whether emergency attention at MHCAS or elsewhere is needed.
If advised to attend MHCAS then no referral is needed.
