CICS (Camden Integrated Care Service)

Camden Integrated Care Service (CICS) aims to manage co-morbidities and frail patients with specialists from different disciplines working out of the same location to better support your patients with long-term health conditions.

CICS referrals (see related services, left) can be made for patients with:

  • chronic kidney disease (CKD)
  • memory clinic
  • hypertension.

The CICS for epilepsy, COPD and heart failure are no longer available. Referrals for epilepsy should be sent to secondary care via CHI RSS, unless it's first-seizure epilepsy. For COPD and heart failure, please see the information letter.

How to refer

EMIS form

Referral methods: e-Referrals

Via e-Referrals to CHI RSS:

Complete the CICS Proforma in EMIS.

Form location: EMIS Forms > Camden Integrated Care Service Referral Form

Create an e-Referral via your GP clinical system.

In e-Referrals, select Specialty/Clinic Type: General Medicine, then Service: Camden Integrated Care Service (CICS).

Send UBRNs (Unique Booking Reference Numbers) to CHI RSS.

Referrals will then be entered onto EMIS Web by CHI RSS and then viewed and triaged by the respective discipline. All referrals will be reviewed by a specialist, leading to one of the following outcomes:

  • onward hospital referral
  • community assessment
  • virtual assessment with a management plan feedback to the GP
