The sleep and ventilation service at the Royal Free Hospital has updated its Adult Sleep Disordered Breathing Referral Form to support appropriate patient triage. The updated form is available on EMIS.

Please note: patients may be contacted to arrange testing prior to an appointment. Those who do not attend testing within a specified timeframe may be discharged and will require re-referral.
