The GP Provider Alliance, working closely with the NCL ICB and NCL Health Alliance, is seeking primary care representation from each borough to become members of the four Long Term Conditions (LTC) Clinical Networks:

1. Respiratory
2. Cardiovascular disease
3. Chronic kidney disease
4. Diabetes

Each network meets once a month. Primary Care views on the relevant areas of work are vital to the success of the networks. Clinical network members are expected to represent the views of their respective borough. There is no expectation for interested members to deliver any of the work. Interested colleagues can apply to become members to more than one network, so long as attendance is confirmed. 

All candidates wishing to support system work on the NCL LTC Clinical Networks are invited to complete the expression of interest form. Completed forms are to be returned to Irodotos Nikolaidis, Irodotos.nikolaidis@nhs.net by Tuesday 15 April 2025.
