A new Annual Health Check template has been introduced for GP practices across North Central London ICB to streamline and monitor the completion of health checks for individuals with learning disabilities and autism. 

More specifically, the template aims to:

• standardise and simplify the documentation required for annual health checks
• reduce discrepancies and variations across NCL and within boroughs
• encourage GPs to complete 80%+ of the template
• improve the quality and the usefulness of the data extracted. 

The new template now highlights: 

• reasonable adjustment flag
• national screening
• lifestyle
• medications review
• examinations and investigations 
• vaccinations 
• functional life skills

The template is designed to ensure that GPs maintain a consistent and accurate record aiming to address health inequalities and ensure that vulnerable people within our population receive the right level of care needed.
