The NHS England Register with a GP Surgery service is available to all GP practices in England. It provides an online registration option for patients, helping to reduce the administrative burden for general practice and making GP registration more accessible to the public. The 2024/2025 GP contract mandates the use of the ‘Register with a GP surgery service’ in both digital and paper form by October 2024.

NHS practices in England can sign up in NHS Profile Manager to start using the Register with a GP surgery service. The process takes around 10 minutes to complete.

Updated paper registration form

An updated paper form is now available to all practices in England offering the national Register with a GP surgery digital registration service.

The new-look form uses the same standardised question set as the digital service, reducing variation in the patient experience, and has been designed using feedback from patients and practices.
