General practices are warmly invited to the second Building Health Partnerships session for the North London STP. The session is an exploration and re-imagining of self-care and social prescribing in North London that brings together local residents and the voluntary sector to work with the NHS and local government. With a focus on groups of GP practices called Care Closer to Home Integrated Networks or CHINs, the session will look at how different people and organisations come together in North London to create a system of care in communities – looking at what works locally and across North London.
