NCL WidePCN DES Assurance
The ‘PCN DES 2023-24 Good Practice and 2024-25 Approach’ supports NCL PCNs deliver the 2024-25 Network Contract Directed Enhanced Service requirements by:
- Highlighting 2023/24 PCN DES delivery good practice and learning
- Setting out the timetable and reporting process for 2024/25 PCN DES
Action required by PCNs
PCNs are asked to demonstrate service delivery through completing six short online surveys – one for each area of the DES. The approach to have six short online surveys rather than one combined online survey will enable PCNs to provide assurance of each service area in a more manageable manner and at separate points in time rather than having to complete one big survey in one go. The surveys can be accessed via:
- Structured Medication Review (SMR) & Medicines Optimisation
- Early Diagnosis Cancer
- Personalised Care & Social Prescribing
- Tackling Health Inequalities
- CVD (Prevention & Diagnosis)
- Enhanced Health in Care Homes & Proactive Care (for Frailty)
PCNs are asked to complete each online survey by 5pm, Monday, 31st March 2025.
