NHS Health Check

This locally commissioned service (LCS) is designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia. The aim is to reduce avoidable premature mortality and morbidity through early identification and management of risk factors and early detection of disease.

Eligibility criteria: 

  • fully registered with a Camden GP
  • aged 40-74 years (i.e. the NHS Health Check must be completed on or after 40th birthday and before 75th birthday)
  • not have a diagnosis of documentation of:
    • coronary heart disease
    • chronic kidney disease (CKD stages 3-5)
    • diabetes 
    • previous stroke
    • hypertension
    • atrial fibrillation 
    • transient ischaemic attack (TIA)
    • heart failure 
    • peripheral arterial disease
    • familial hypercholesterolemia

The NHS Health Check includes:

  • a cholesterol blood test
  • the collection of additional risk assessment data to calculate a QRISK2 score (a score indicative of a person’s risk of developing heart attack and stroke in 10 years)
  • assessment of the risk of type 2 diabetes with a confirmation blood test for those at high risk (with QDiabetes score >5.6). Patients below the high-risk threshold can be referred to the Diabetes Prevention Programme based on clinical assessment
  • a manual pulse check for patients aged 65+ to assess for presence of atrial fibrillation
  • administration of the Audit-C screening tool for alcohol consumption and referral to the Camden Drug and Alcohol Service if required
  • the communication of the risk score to patients, including a full explanation of results, tailored to the individual needs of the patient
  • lifestyle advice and risk factor management and medication as appropriate, including referral to appropriate community weight management, alcohol support and stop smoking lifestyle services
  • raising awareness of dementia and signposting to memory clinics for patients aged 65+
  • patients can be referred to the community mental health and wellbeing service (iCope and WISH+) and Drug Misuse Public Health LCS if such support is needed. List of practices signed up to deliver the Drug Misuse LCS is attached
  • movement onto disease registers and follow up as required.
