Speech and Language: CLCH

The Planned Care Speech and Language Therapy Service provides assessment, treatment, support and advice to adults with acquired communication and/or swallowing disorders. This includes all aspects of communication (including speech, language and voice) and swallowing disorders.
Eligibility Criteria
Inclusions
- aged 18+
- registered with a Barnet GP
- has an acquired swallowing and/ or communication disorder
Exclusions
- Patients with no rehabilitation potential including lack of cognitive ability to follow communication rehabilitation goals and/or not motivated to participate in rehab.
- Patients who are not medically cleared for discharge and are not likely to be in the near future.
- Acute exacerbations of illness that requires an acute hospital environment to deliver care needs.
- Patients with long term complex care needs e.g. neuro rehabilitation
- Patients whose primary diagnosis / area of concern is mental health or social care related.
- Patients presenting with a lisp.
- Patients presenting with a hearing impairment.
- Patients presenting with a primary diagnosis of head & neck cancer.
- Patients with Parkinson's disease (refer to Parkinson's service)
- Patients with enteral feeding requirements (refer to Home Enteral Feeding team).
- Patients with a voice problem who have not had an examination of their larynx by an ENT specialist
- Training in communication and dysphagia intervention will not be provided to nursing homes.
