Reablement Service

The aim of the Reablement Service is to support an individual to reach the maximum level of independence possible following a period of ill health or significant change in circumstance.

The service lasts from a few days to a maximum of six weeks and is free of charge to the person using the service.

The team comprises of Reablement coordinators, social worker, physiotherapist, occupational therapist, rehabilitation assistants, pharmacists and enablers who deliver the Reablement care to people in their homes.

The principle is to promote independence by supporting service users to carry out tasks and work towards their individual goals. The aim is to reduce the need for longer-term care and assist users in remaining at home for longer and to live more independent lives.

The service supports various activities of daily living, such as:

  • personal care
  • meal preparation
  • shopping
  • medication management
  • improving indoor mobility
  • accessing the community.

The service can provide a maximum of four visits per day.

If ongoing support is needed after the period of reablement, formal care can be arranged dependent on eligibility, and a financial assessment is completed.


Eligibility Criteria

Inclusions

Patients who are:

  • an Islington resident
  • over 18
  • require a support package at home (such as a domiciliary care package) 
  • lack confidence and/or require support after a health or social care change, such as illness, deterioration in health or injury
  • have difficulty in performing essential daily living activities (e.g. personal care tasks, mobility, medication management, meal preparation and shopping) and have potential to increase independence in these activities
  • motivated to actively participate with the Reablement service and work towards agreed goals and outcomes
