Paediatric Community Asthma Nurse

The Paediatric Community Asthma Nurse supports children aged five to 18 years with an asthma diagnosis.

The nurse will complete three consultations, providing both asthma education and support. Patients are seen face-to-face in either a clinic or through home visits. Phone reviews and video consultations will also be made.

Eligibility Criteria

Inclusions

The patient must:

  • be aged five to 18.
  • have a diagnosis of asthma
  • have an Enfield GP.

How to Refer

Referral methods: Email

Clinicians should refer to the Community Paediatric Asthma Nurse for further education and support if:

  • poor compliance
  • social factors affecting control
  • <19/27 ACT (asthma control test)
  • history of allergy/hayfever affecting asthma.

Please include the following in your referral:

  • brief summary of asthma review
  • last 6 months of prescription requests.
