The Multidisciplinary Falls Clinic provides expert assessment, advice, and prevention strategies for older adults at risk of falls. This is service is for clinicians and GP referrals only – self-referrals are not accepted.

What the service offers: 

  • comprehensive falls risk assessment by a Consultant, Occupational Therapist, Physiotherapist, Nurse, and Rehabilitation Assistant
  • personalised falls prevention strategies
  • signposting to additional support services

The MDT falls clinic response time is 2 to 6 weeks time. Urgent referrals requiring falls assessment, prevention and intervention should be referred to Haringey Urgent Response and Recovery Team (HURRT).


Eligibility Criteria

Inclusions

  • aged 65+
  • resident of Haringey
  • history of falls, trips, slips, or balance problems
  • fear of falling or lack of confidence with walking
  • able to attend the clinic independently or with assistance from one person
     

Exclusions

  • medically unstable such as acute infection or confirmed syncope cases
  • newly diagnosed or active neurological conditions (e.g., stroke)
  • severe mental health conditions preventing engagement in therapy
  • requires urgent intervention due to recent multiple falls

Complete the Islington & Haringey Adult Community Services Referral Form and send to haringey.adult-referrals@nhs.net

  • select the Falls Prevention box under Haringey Locality Therapies
  • in the Reason for Referral section, specify if the patient requires input from the Multidisciplinary Falls Clinic. Otherwise, they may be directed to community falls services for a home-based assessment.
