The North London NHS Foundation Trust (NLFT, a merger of BEH and CANDI) Islington Assertive Outreach Team (AOT) provides care coordination, recovery-focused interventions, psychological, practical and emotional support, motivational interviewing and Mental Health Act expertise. 

The team will work with patients on recovery goals, provide regular medication reviews and help service users with a range of problems such as substance misuse, housing and benefit issues, emotional well-being and extra support in times of crisis.

The service is for those:

• aged 18-65
• with a history of psychotic disorders and complex needs, and who may have had difficulty working with other teams or services and may have had many admissions to hospital.
