In-Patient Units, Edgware Community Rehabilitation Unit, Marjory Warren & Adams Unit: CLCH
BarnetThis service aims to support patients to manage their own health conditions and best look after themselves.
Patients will have therapy input in the rehabilitation units to help them maximise their recovery and become as independent as possible. Nursing care and therapy is provided on the units.
Patients must be medically optimised, so they can complete a short period of necessary assessments to develop their care plans, enabling them to discharge to an appropriate safe discharge destination and have ongoing rehabilitation in the community as appropriate.
Both the CLCH rehabilitation unit sites have access to senior doctors from Monday to Friday, who will be available as needed. A consultant makes twice weekly visits to the rehabilitation units.
Out-of-hours medical cover is provided by a local GP service.
Eligibility Criteria
Inclusions
- Patients who have rehabilitation needs
- Patients who require additional assessment needs before being discharged to an appropriate destination.
Exclusions
- Patients that are not registered with a Barnet GP
- Patients that are medically unwell
- Patients that are unable to participate in rehabilitation process due to level of cognitive impairment
- Patients that have no rehabilitation goals
- Patients under the age of 18.
