In-Patient Units, Edgware Community Rehabilitation Unit, Marjory Warren & Adams Unit: CLCH

Therapy input and nursing care are provided in rehabilitation units to support patients in maximising recovery and independence. Patients must be medically optimised to complete assessments to inform care plan development, to enable safe discharge into the community.
Eligibility Criteria
Inclusions
- Patients who have rehabilitation needs
- Patients who require additional assessment needs before being discharged to an appropriate destination.
Exclusions
- Patients that are not registered with a Barnet GP
- Patients that are medically unwell
- Patients that are unable to participate in rehabilitation
- Patients that have no rehabilitation goals
- Patients under the age of 18.
