The Diabetes Integrated Practice Unit provides support for adults with type 2 diabetes across Tier 1 primary care, Tier 3 community clinics, and Tier 4 hospital services.
The service offers a multidisciplinary support with a focusing on preventing hospital visits and long-term complications. It provides virtual consultant advice for complex cases, structured education sessions, dietary consultations, pre-conception counselling, and psychological support.
Eligibility Criteria
Inclusions
- resident of Camden or registered with a Camden GP
- type 2 diabetes
- disengaged type 1 diabetes

Referral methods: Email
Referrals for the Tier 3: Community clinic, complete the Camden Community Diabetes IPU Referral form and send to the CNWL Central Access Team: camdenreferrals.cnwl@nhs.net
Referrals for the Tier 4: Hospital services, send via e-RS.
