COPD Domiciliary Respiratory Service: CNWL

Camden

A consultant-supported multi-disciplinary team, providing domiciliary visits to housebound patients with COPD, who frequently have exacerbations or hospital admissions.

The team also provides:

  • exacerbation management in previously known patients
  • support on hospital discharge (with diagnosis of COPD or likely diagnosis and within 72 hours of discharge). Please note that this service is not designed to address acute medical needs.
  • medication optimisation and medication trials (e.g. up-titration of ICS)
  • individualised self-management plans e.g. breathlessness management, secretion clearance, disease and exacerbation education.

Eligibility Criteria

Inclusions

  • housebound patients with confirmed diagnosis of COPD
  • and/or frequent exacerbations or hospital admissions
  • patient must have a Camden GP
     

Exclusions

  • Patients that can attend a respiratory clinic will be offered all the above in an out-patient setting
  • Patients without a COPD diagnosis

Complete the Camden Integrated Adult Community Services - CNWL referral form and attach any relevant discharge, outpatient reports or recent spirometry.
