Diabetes Integrated Practice Unit

Camden

The Camden Diabetes Integrated Practice Unit (IPU) offers the following services.

Tier 1: Primary care support

  • regular meeting with practice diabetes leads and clinicians
  • identify priority practices and support through meeting, education and MDTs

Tier 3: Community clinic

The Camden Community Diabetes Service is made up of a multi-disciplinary team (MDT) providing support to adults with (mainly) type 2 diabetes in the community to manage their condition.

The team helps patients with complex health issues who need multiple therapies and the aim is to avoid hospital attendances and prevent risk of long-term complications. Patients are provided with the skills, knowledge and confidence to self-manage their diabetes effectively.

The following services are provided:

  • Consultant non-urgent advice for complex cases: virtual review of EMIS records and management plan completed for your patient (please specify details below: e.g. poor glycaemia, BP or cholesterol etc).
  • Diabetes structured education sessions (DESMOND/Bengali education) for those with type 2 diabetes diagnosed within the last year (confirm two readings of HbA1c ≥48 mmol/mol or how other diagnostic criteria met) or 1-2-1 with dietitian if patient not suitable for group education. 
  • Diabetes management criteria for GLP-1: maximum tolerated dose of three hyperglycaemic agents with a HBA1c >58 mmol/mol or agreed target and BMI >35 (BAME BMI > 30). 
  • Diabetes management with HbA1c above target for those already on injectable therapy despite lifestyle intervention and for injectable therapy titration or recurrent hypoglycaemia.
  • Diabetes dietetic input (HbA1c > 58).
  • Pre-conception counselling: Type 2 diabetes for women of childbearing age planning pregnancy in the next six months.
  • Psychological support for diabetes related distress.

New patient and follow-up appointments will be made by the service's administration team at St Pancras Hospital for clinics at St Pancras Hospital or Belsize Priory Health Centre.

Tier 4: Hospital services

  • type 1 management and education
  • gestational diabetes
  • insulin pumps
  • CKD stage 4 and 5 with raised Hba1c
  • emergency podiatry referrals (ulcer, infection, necrosis, absent pulses, charcot).

Available at Royal Free Hospital or University College London Hospitals.


Eligibility Criteria

Inclusions

  • resident of Camden or registered with a Camden GP 
  • type 2 diabetes 
  • disengaged type 1 diabetes
