Sarah Milne, Kidney Nurse Consultant at Royal Free Hospital and lead for the NCL CKD Integrated Service would like to invite primary care clinicians across NCL to complete a short online questionnaire as part of an MSc research study. 

The purpose of the questionnaire is to evaluate the impact of a new referral pathway on service users, as part of a wider expansion to the CKD service in NCL. 

The questionnaire takes less than 10 minutes to complete and will be used to gather your opinions on your experience of using the new referral pathway.
