It has recently been highlighted there has been a noted increase in submissions of incomplete X-ray referral forms. Finchley Memorial Hospital Community Diagnostic Centre (CDC) would like to remind referrers to kindly ensure that referral forms are completed clearly and fully. 

For patient safety, governance, and Ionising Radiation (Medical Exposure) Regulations 2017 (IR(ME)R) compliance, it is mandatory that all referrals include the full name of the referring clinician (printed clearly) alongside any signature and GMC/professional registration number. The CDC is therefore kindly requesting that T-quest forms are accurately completed to potentially avoid patients being turned away on the day of their appointment.
