NHS Diabetes Prevention Programme update: Use the self-referral pathway

The Healthier You NHS Diabetes Prevention Programme (NDPP) is a 9-month behaviour change programme. It supports people with non-diabetic hyperglycaemia or a past diagnosis of gestational diabetes to live healthier lifestyles and reduce their risk of developing type 2 diabetes. 

In Camden, GP referrals to NDPP are low compared to other NCL boroughs. 

Practices are encouraged to refer patients via the new self-referral pathway, through sending an Accurx SMS message to eligible patients with the self-registration form link

Across NCL, NDPP is delivered by Living Well, Taking Control (LWTC) via face-to-face and tailored remote groups and digitally by Liva Healthcare. LWTC offer 1:1 support and webinars for GP practices, which staff are encouraged to attend.
