New GP Feedback & Alert system to replace Quality Alerts from 9 March

From 9 March 2026, a single route for GPs to raise concerns with providers will replace all Quality Alerts and GP liaison clinical emails. 

To find the form

Go to EMIS > Documents > NCL Global > GP Feedback & Alert Form

When to use the new EMIS form

Use whenever raising a concern with a provider (including inappropriate requests/workload transfer). 

How to use the form

  • Complete all sections marked * otherwise it will be automatically returned 
  • Choose the escalation level based on seriousness and risk:
    • Feedback direct to service
    • GP Alert 
    • Patient Safety Event 
  • Save form into EMIS record and send as an attachment (NOT as a PDF; the automated system can’t read so a PDF will bounce back)

You will receive automatic email confirmation and email outcome reminders.

Feedback/alert automatically escalated if no response; no chasing is required. 

The system is being piloted live by an NCL primary care network in Barnet, with early feedback that GPs are receiving quicker and more helpful responses from services.
