LTC LCS: Key performance indicator guidance for practices and PCNs

NCL ICB have provided instructions for NCL practices and PCNs on how to meet KPIs 4 and 5 of the Long Term Condition LCS specification for the 2024-25 year. The deadline for practices and PCNs to complete this work is Wednesday 30 April. 

KPI 4 instructions

PCNs are asked to complete an 8-question online form, which asks for a summary of your weighted payment project so far, including any outcomes, observations and challenges. The KPI 4 online form is to be completed at PCN-level (not by every practice).

KPI 5 instructions

Practices are asked to complete a 7-question online form, which asks how many CollaboRATE survey responses were captured, what the feedback has been, what objectives were agreed at PPG discussions, and 4 examples of qualitative feedback given on the forms. The KPI 5 online form is to be completed at practice-level.
 
KPI 5 asks practices to produce a report. To clarify, practices must complete the above online form to meet the KPI, and the ICB does not need to see a report.
