NCL ICB Primary Care Team invite all practice staff who are involved in delivering the Long Term Conditions Locally Commissioned Service (LTC LCS) to participate in a short mid-year survey regarding the use of the MYCaW tool. The team would like to hear your feedback about your early experience of the tool so far, to help understand how the tool can be best used in the LTC LCS.  

The survey will take approximately 5-10 minutes to complete and will close on Tuesday 29 October 2024. All responses will remain confidential.
