Learning disabilities health checks: Use updated annual pre-check questionnaire

An updated pre-check questionnaire for use in learning disabilities (LD) annual health checks across north central London has been co-produced with people with lived experience of LD and systems partners.  

The purpose of the pre-check questionnaire is to enable people with a learning disability, and their carer(s), to think, plan and prepare for their LD Annual Health Check (AHC). This allows the clinician to focus on the most important health needs, make more informed recommendations, identify potential problems early and ensure the annual health check is personalised to the individual’s needs and specific concerns.

The questionnaire helps the clinician save time during the health check by allowing the patient to prepare information beforehand, as well as enabling the patient to have increased independence and control over their own health.

The NCL AHC pre-check questionnaire was adopted across north central London in December 2024. It has now been updated to reflect recent changes in the age eligibility of some cancer screening programmes, and to include the lung cancer screening programme. It has also been embedded into EMIS.
