From the 17 to 21 March, the following forms were rolled out remotely by the ICB under NCL ICB Resource Publisher (shared folders).

Barnet roll-out

Please note the following form was removed and updated version is now located under the NCL Global Documents folder in the Infectious and Tropical Diseases Sub-folder with Reasonable adjustments codes added:

  • Infectious and Tropical Diseases > Outpatient Parenteral Antimicrobial Therapy referral – Barnet & Chase Farm 

Enfield roll-out

Please note the following form was removed and updated version is now located under the NCL Global Documents folder in the Infectious and Tropical Diseases Sub-folder with Reasonable adjustments codes added:

Please note the following form was added to ISL Global Documents:

  • Mental Health > Bereavement > Islington Bereavement Service Referral – St Joseph’s Hospice First Contact Team
