Since Monday 1 December, the Central London Community Healthcare (CLCH) Stoma and Bladder & Bowel services have aligned to the CLCH referral process via Single Point of Access (SPOA).

Referral forms are already available and up-to-date on the NCL General Practice website.

All forms will need to be sent to the CLCH Planned Care Team. No requests will be processed unless received via SPOA.
