PCN Advisor Connect and Share: Spotlight on Health and Wellbeing Coaches

This session, ran by London PCN advisors Hina Shahid and Lianna Martin, will help bring clarity on the value of health and wellbeing coaches in primary care and share best practice examples.

The PCN advisor role is an NHSE associate role in place to support PCNs to embed the three personalised care roles (social prescribing link workers, care coordinators and health and wellbeing coaches).

Health and wellbeing coaches are a powerful asset to motivate and coach patients to work through issues and behaviors impacting their health and wellbeing.

This session aims to:

  • provide clarity on what the health and wellbeing coach role is and how it is different to a social prescribing link worker and care coordinator; what is their superpower
  • share best practice examples of how health and wellbeing coaches are delivering tailored support for patients and tackling health inequalities
  • demonstrate how they are working with social prescribers and care coordinators
  • share how best to support and manage health and wellbeing coaches
  • sign-post to resources for best practice in inducting and setting the direction and managing care coordinators.
