PCN Advisor Connect and Share: Population Health and the PC ARRS Roles

The personalised care ARRS roles are a powerful asset to work with populations that are experiencing inequalities in health and healthcare outcomes, experience or access.

This session aims to:

  • Define what population health and population health management is
  • Explain how population health and personalised care fit in the health inequalities agenda
  • Illustrate how population health can inform, develop and innovate social prescribing, coaching and care coordination
  • Give examples of where this has been a success, using communities as assets not deficits
  • Give practical next steps of how you can start with population health and build the relationships across your neighbourhood to develop this
  • Show useful tools to understand population need including socio-economic needs, through a personalised care lens

This is part of a monthly connect and share series by the PCN advisors. 

It supports primary care to embed the three personalised care ARRS roles (social prescribing link workers, health and wellbeing coaches, care coordinators).
