To support practices in the delivery of the Long Term Conditions Locally Commissioned Service (LTC LCS), NCL Training Hub will be joined by HealtheIntent to demonstrate and explain:

• the benefits of HealtheIntent case finding and how this supports a population health approach
• how to access HealtheIntent
• how to navigate the case finding dashboards to show different populations and priority groups
• how to generate a patient list looking across multiple conditions.

GP IT will also demonstrate how to use the EMIS case finding searches.

An alternative webinar will take place at 1pm on Thursday 14 September. Please note that you only need to register for one of the sessions as the same content will be covered on both dates.
