This open forum is for clinicians and GP practice teams to discuss any aspect of the Long-Term Conditions Local Commissioned Service (LTC LCS).

The session will include a reminder of 2026/27 KPI requirements and updates to the LTC LCS, with plenty of time for questions and shared learning.

The NCL Change Management team will be on hand to provide guidance, answer questions and share best practice. From questions relating to templates, searches, recall processes, Model of Care queries, KPI delivery, or any other aspect of the LTC LCS.
