About this event

In this webinar we will focus on the tools and resources available from UCLPartners and the British Heart Foundation to help primary care teams restore routine care for patients at high-risk with cardiovascular conditions during and post-Covid. Resources shared will include UCLPartners' proactive care frameworks based on new pathway development, virtual consultations, digital solutions and optimal use of the wider primary care team.
