Join NHS England's Primary Care Transformation team in exploring what is meant by signposting and what the barriers to this being an effective tool in practice can be. 

The team will share some quick wins that are working well across the country and will invite those attending the session to share their experiences. There will also be demonstrations of tried and tested tools to help and practices develop and adapt ideas that work for them.

Learning outcomes include:

• able to identify the barriers to signposting
• able to begin addressing some of the common problems from the ideas and learning shared
• able to use quality improvement tools to determine whether a change leads to an improvement.
