Personalised Care Planning Training Programme - Year of Care Partnerships

The core training aims to ensure that participants understand the Year of Care approach to care planning. The training not only focuses on the attitudes and consultation skills to deliver a collaborative care planning consultation but also shares and integrates into training the tools and resources to aid the practical implementation of care planning.

This training will be for GPs, HCAs, Practice Nurses and any other clinical staff. Staff must attend both Day 1 and Day 2.

Lunch served from 12:30 - 13:00
