Care Coordinators Support Session

A new London care coordinator support session is taking place on Thursday 25 August from 1-2.30pm.

This session will support new care coordinators as they start their role.

It will cover the following:

  • The context of care coordinators in London: what is their importance for primary care and beyond?
  • Practice, PCN and ICS structures: where you fit
  • What support is available to you
  • Care coordinator work and role examples; with top tips from a care coordinator

The first hour, which is mainly information given, will be recorded, edited and shared with new care coordinators and colleagues across primary care. 

At the end of the one-hour session, there will be 30 minutes for Q&A and networking open to all London care coordinators. 

The plan following this event is to use the edited New Care Coordinator Support video as resource for all PCNs to use to share with their new care coordinators, this will be complemented by quarterly pan London drop-in sessions for new care coordinators to ask questions and network.
