The GP and hospital forum started as a three-month pilot in October 2021 to create a space where clinicians and managers could come together with primary care colleagues from across north-west London.

The pilot was a collaboration with the Imperial College Healthcare NHS Trust, Hammersmith and Fulham (H&F) borough director, the co-chair of H&F LMC and Chelsea and Westminster NHS Foundation Trust and was for H&F borough GPs only.

It opened to all eight boroughs across north-west London in January 2022 as a monthly meeting where GPs, partners, and trust clinicians could identify emerging issues and shape new ways of working. Services presenting at the forum include the Cerner and Rego teams, ED, and specialties such as cardiology and dermatology. Topics included GP letters, outpatient improvements and new discharge processes.
