Better: Universal Care Plan Online Access

The Universal Care Plan (UCP) is a digital care plan based on what matters most to your patients. It allows their wishes, along with your individual care and support needs, to be shared digitally with healthcare professionals involved in your care while living in London.

The UCP supports Londoners with end-of-life care as well as other long term conditions including dementia, frailty, sickle cell disease, learning disabilities and autism. Carer contingency plans can also be recorded using the UCP and it can be used throughout a person’s entire lifespan, from childhood through to adulthood.

Soon, patients will be able to edit their own universal care plan via the NHS app or online platform. This feature is being piloted, view access only is available at present.

This web platform allows access to all current UCP assigned to a practice as well as their last review dates.
