The aim is to support GP practice teams in the management of type 2 diabetes in patients aged 18 to 40.
The training included:
- Insights from specialist consultants on complex cases
- Practical steps for clinicians
- Implementing learnings into daily practice
- Reimbursement incentives for sharing knowledge within teams
Reimbursement to share learning amongst the team will be allocated to practices based on the size of the patient cohort. This will be a minimum of £100, increasing for areas with larger cohorts.
Following this training session, you must complete this feedback form to obtain a completion certificate which will flag that the training has been completed and payments need to be arranged.
