On behalf of WNL ICB, Amy Bowen, Director of Proactive Care and Long-Term Conditions, and Katie Coleman, Deputy Chief Medical Officer, would like to sincerely thank all primary care networks (PCNs), practices, clinical teams and supporting colleagues for their continued commitment to the Long-Term Conditions Locally Commissioned Service, and all their hard work and collaboration throughout 2025-26. 

Delivering sustained improvements in hypertension control requires consistent effort, strong leadership and effective teamwork, and the collective focus across the system has been instrumental in achieving these outcomes.

They are pleased to confirm that all PCNs across North Central London (NCL) have successfully achieved the Hypertension (HTN) Good Control outcome target at the end of year 2025/26, demonstrating sustained improvement in population health outcomes and strong system wide delivery.
