NHS England’s Type 2 Diabetes Local Data Packs 2024-25 provide primary care teams clear, practical insight into local population need and diabetes care activity. They help practices and primary care networks identify variation, target improvement, and plan care more effectively. With delivery of the eight care processes now reflected in both Quality Outcome Framework and the NHS Oversight Framework, the packs support provision of more consistent, high quality care.

Primary care teams are encouraged to access and use the packs via the FutureNHS webpage (log-in required).
