Communication happens all the time, but does not always seem to get the desired outcomes. In this module, NHS England's Primary Care Transformation Team will explore why that happens and what effect it can have, before introducing participants to the SBAR tool, which can help avoid miscommunication.

• describe the components of the SBAR communication tool
• practise using the tool
• identify and explore practical applications for SBAR in practice.
