Improving the Health of Patients with CKD LCS

This borough-level locally commissioned service (LCS) will end on 30 September 2023, it will be replaced by the NCL-wide Long Term Conditions LCS on 1 October 2023.

The aim is to improve identification, early diagnosis and management of chronic kidney disease (CKD) in primary care and patient outcomes.
