The aims of the High-Risk Drug Monitoring service are: 

• To ensure relevant monitoring in primary care of selected high-risk drugs is undertaken in line with national and local guidance. 
• To ensure the safe prescribing in primary care of selected high-risk drugs in order reduce potential harm to patients. 
• To ensure patient education and counselling on the selected high-risk drugs is provided throughout the patient’s treatment
