EMIS form

Referral methods: e-Referrals

Please complete the EMIS form and send to CHI RSS via the e-Referrals system.

Please include patient height, weight, oxygen saturation levels and Epworth Sleepiness Score with referral.

If you have any questions, please email rf-tr.respiratoryphysiorfh@nhs.net (Monday to Friday, 9am-5pm).
