Whittington Health Adult Community Nutrition & Dietetics is pleased to announce that they are accepting self-referrals for patients registered with a Haringey GP that meet the referral criteria. Please direct patients to the website (in Resources) to complete the online self-referral form.

The form needs to be fully completed with a clear dietetic reason to help reduce any delays/rejections, including height and current weight, as well as any relevant blood test/investigation results. Patients who are referred will be given the options to have either virtual or face-to-face consultations with a dietitian.


Whittington Health's (WH) Nutrition and Dietetics Service provides effective, evidence-based nutritional assessment, dietary advice and support to patients, their carers and staff involved in their care. The aim of the service is the prevention and/or treatment of nutrition-related conditions, helping local people live longer, healthier lives. 

A range of conditions are managed including obesity, diabetes, IBS and nutrition support.

Eligibility Criteria

Inclusions

Summary of adult inclusion criteria:

  • malnutrition
  • nutrition support
  • pressure ulcers
  • type 2 diabetes
  • pre-diabetes
  • diagnosed digestive issues (IBS)
  • cardiovascular issues
  • weight management
  • diagnosed nutritional deficiencies.

Summary of child inclusion criteria (>18 months, Haringey only):

  • obesity >91st centile
  • constipation
  • fussy eating
  • failure to thrive
  • diagnosed single food allergy
  • diagnosed nutrient deficiency.

Exclusions

  • referrals with insufficient information to ascertain nutrition status (e.g., recent weight, weight history, height)
  • IBS referrals that have not completed the IBS referral proforma (see How to refer)
  • type 1 diabetes/type 2 diabetes on insulin
  • children with complex special needs (e.g., autism, downs syndrome, learning disabilities)
  • adults with suspected or confirmed eating disorders.

Self-referral
