For patients with a diagnosis of heart failure.

This service helps patients manage their condition, encourage early recognition of their symptoms and reduce preventable hospital admissions by providing them with a comprehensive assessment and individualised care plan.

Eligibility Criteria

Inclusions

  • 18+ registered with Camden GP
  • diagnosis of heart failure from secondary-care setting and evidenced by ECHO

Exclusions

  • not suitable for people who require diagnosis or complex interventions that are normally only available from secondary care
